i always can't concentrate on my work, when i doing my work my mind was in the cloud. i imagine that i'm waring with beautiful cloths i designed, going holiday with my friend, buy a lot of thing after i get my salary. it make me very hard to accomplish my work, and there always some mistake in every thing i done.

Maybe your work is just really boring.
Maybe you should do some research on concentration disorders like ADHD, but don't jump to any conclusions. Talk to a proper psychologist.
